CBNA’s Community Service form has been UPDATED for 2023-2024!

Our new form includes a list of approved community service locations that NO longer require “pre-approval” prior to volunteering! If you are planning to volunteer for any of the locations listed on the form, you just need to complete the top portion of the form and have a representative of the non-profit organization complete the bottom portion after you have finished your service hours. Afterward, complete your summary on the next page and turn it into your school counselor so they can record your hours within Infinite Campus.

If you community service location is not listed above, you can still volunteer by getting pre-approved prior to volunteering. You can do this TWO ways:

  1. Submit a copy of the paper form either by email (scan/fax) or stop by the main office for a signature. Your form will be approved while you wait! This is the quickest way to get pre-approval.
  2. Click on this link to submit an electronic request. Please allow up to 3 business days for electronic submissions to be approved and sent back. DO NOT START your service hours until you have received your approved form.

As a reminder, all community service must be performed for non-profit organizations/agencies. Here are a few ideas to help get you started:
